Coordinated Family Support (CFS)

Coordinated Family Supports (CFS) is a service designed for adults receiving regional center support who prefer to remain in their family home. CFS services aim to enhance fair access to services and address racial and ethnic disparities in service purchases. CFS is customized to meet the specific needs of both the individual and their family, delivered in a way that honors their language and cultural background. This support is primarily offered within the consumer’s home environment.

Our CFS program grants consumers personalized assistance with the following plus more:

  • Assisting and training the consumer and their family in navigating comprehensive services and supports tailored to their unique needs, and creating strategies to overcome barriers to accessing general and specialized resources

  • Assisting with the scheduling of service appointments, including medical visits and other necessary engagements

  • Identifying available transportation services or options

Staying Connected

At Connected Care, we believe that connection and community are key to well-being. To foster meaningful social interactions, we organize fun and inclusive monthly activities for neurodivergent individuals to meet, connect, and build friendships in a relaxed environment through our Connected Care Programming.

From game nights and art workshops to outdoor adventures and themed social events, each activity is designed with accessibility and sensory needs in mind. Our experienced staff creates a welcoming atmosphere, encouraging participants to engage at their own comfort level and pace. These gatherings offer a safe and supportive space for individuals to explore new interests, strengthen social skills, and enjoy a sense of belonging within a community that values and celebrates neurodiversity.
